This free blood pressure excel template helps you to keep record of your heart rate and diastolic and systolic blood pressure. On top of that, your data will be automatically evaluated by highest, lowest and average values over the complete year.

How does the Excel Template for Blood Pressure work?
The only hard thing is to make and mark all your blood pressure data and put it into the Excel template. The usage of the template is very easy:
- Download the file and open it with Excel (or any other table calculation software).
- On the spreadsheet named “overview” put in your name and the correct year.
- All other data on that sheet is coming from the monthly sheets – please DO NOT delete them.
- Instead open the sheets month by month and delete the demo data I put in.
- Then switch to the current month and fill in the rows and columns with your heart rate, diastolic and systolic values from morning, noon and evening.
- The Excel template will automatically generate a line chart in the lower part of the sheet for all data.
- If you have any notes to make, use the big field on the right. Maybe you had fever or other health related issues your doctor might want to know.

The next sheet is “January”, where you either delete all data or fill it up with your already measured data.

Dear Sir,
I am in my seventies and I have hypertension and on medication. I have an OMRON BP monitor and take several readings a day. I have been doing this for the past 3 years.
I have searched the internet several times over the years but unable to find a simple easy to use template that I can record my readings and also view them as a chart. At last I came across your chart about a week ago. I like it and it is user friendly.
